PATIENT QUESTIONNAIRE FREQUENTLY ASKED QUESTIONS

This page answers any questions you may have on how Cardinal handles and uses the responses provided to our Questionnaire when it collects such information from you or from a third party.

Why am I receiving this questionnaire?

When you met with a member of the Major Trauma Signposting Partnership (MTSP) in hospital, either as a patient or in your capacity as Next of Kin, you consented to allowing the MTSP to follow up with you after discharge.  A copy of your follow-up consent can be provided as necessary.

How will my responses be used?

Responses provided will be used to help Cardinal evaluate the MTSP and develop their understanding of how patients get on when they leave hospital, and also amend or improve their service where necessary.

Responses will also be used to inform the NHS hospital about their patients’ experiences when they leave hospital.

Will my responses be anonymous?

Yes, responses will be collated and analysed by Cardinal in an anonymous way.  However, Cardinal will be able to identify who has responded via a Unique Identifier code, allowing them to send out reminders only to those who have yet to reply.  Any subsequent analysis of your responses will be anonymous and no personally identifiable data will be shared with third parties.

What if I elect to be contacted for follow-up questions?

If you request a follow-up call to discuss any of your responses in more detail, Cardinal will be able to identify you via the Unique Identifier code.  This will enable them to contact you to discuss the questionnaire further.

Will my responses be made available to anyone else?

No.  Your individual responses will be collated and stored by Cardinal purely for their own information and evaluation.  All the responses will be grouped together and analysed as anonymous responses.  Once the results have been grouped together and anonymised,  Cardinal may publish  and share the results with third parties.

Cardinal does not sell personal information to anyone and only shares it with third parties who are facilitating the delivery of Cardinal’s services or to whom you as the data subject have explicitly consented to sharing your data with.
